Roster & Waiver Guidelines

All teams are required to set their roster on the DiscNW website and all players must sign the required DiscNW waivers before their first game. Please read through the steps below to set your roster, send the e-waivers to your players and check your team's waiver status.



All teams are required to set their roster on the DiscNW website. DiscNW is no longer offering to do this for teams, however, we are happy to help guide you through the process! In order to set your roster you must have a DiscNW account that is connected to the team for which you are setting a roster by being listed as a coach or captain. Please see the two steps below for setting your roster.

Step One: Create Player Accounts

We ask that all coaches please encourage players to make a DiscNW account for themselves. By doing so, this will allow the coach to auto fill the players’ names into the roster and the player’s account will be connected to the team, so they will be able to complete the required waivers online and see their game schedule.


Here are account creation instructions for players:

  1. Create an account for the player. 
    • Go to www.discnw.org.
    • Click “New Account” in the upper right corner.
    • Enter all required information for the parent and click “Create Account.”


Step Two: Create Team Roster

Coaches: If any of your players do not have a DiscNW account, please collect a working email address from those players and enter it as you enter their name on the roster. This will allow you to email them the DiscNW e-waivers.

To complete your team's online roster, follow these instructions:

  1. Login to your discnw.org account and go to your profile by clicking your name in the upper right corner and selecting “Your Profile.”
  2. Click on the “Recent Teams” tab under your basic profile information. This tab shows all of the teams you have captained, coached or played for.
  3. Select the team for which you would like to set a roster. (Please note: your team must be registered for a league or event in order to set an event roster.)
  4. Make sure that the event name is listed correctly.  To change the event listing for your team, use the "Other Events" drop down box next to the event name.
  5. Click the "Roster" tab below your "Team Information."
  6. Click the blue pencil icon on the right, under the "Roster" tab, to edit your roster.
  7. You have two options now:
    • If your team has participated in a past DiscNW league or event, you can copy players from a previous roster for the team by using the "Copy Players from Another Roster" tool.
    • You can add players one at a time by entering the players’ gender, names and e-mails. If a child has an account, their name should auto-fill as you are typing. Please select the auto fill option because then their account will be linked to the roster. If an auto-fill option does not appear, please see "Step 1: Create Player Accounts" above.



DiscNW has three different required waivers: a participation & liability waiver that must be signed by a parent/guardian if the player is under 18 years old, a concussion information waiver that must be signed by both the player and the parent/guardian, and a sudden cardiac arrest awareness waiver that also must be signed by both the player and the parent/guardian. 


If a player has a DiscNW account, then they sign the e-waiver by checking their "Reminders" on the DiscNW website. To find and complete an e-waiver using the "Reminders" feature, the player or parent should follow these instructions: 

  1. Login to the player's discnw.org account.
  2. Click on the bell icon at the top-right corner of the web page, then click on the "Reminders" link.
  3. Look for the "Unsigned Waivers" section and click on the "Sign Now" button.  This will take the signer to the first waiver that needs to be signed.
  4. Read the waiver and type the appropriate name (parent or player) in the box at the bottom of the page to accept the terms of the waiver.
  5. Once the first waiver is signed, the website will automatically navigate the user to the next waiver upon which a signature isrequired, until all waivers have been signed.


If a player does not have a DiscNW account, but provided an e-mail address that was entered when the coach added the player to the roster, then the coach may send a link to the e-waiver through the DiscNW website by following these instructions:

  1. Login to your discnw.org account.
  2. Go to the league webpage.
  3. Click on “Registration” and select your team name in the drop-down menu.
  4. Make sure that you are on the “Waivers” tab. You will see your team roster with a thumbs up next to those who have signed the waivers, and a thumbs down next to those who haven't yet signed or completed the waivers.
  5. Click on the “Send Waiver Reminders” box on the right, above your roster to send a link to the e-waiver to the players who have not yet signed the waivers.
  6. A pop-up window will appear. Please select the players who you wish to send the e-waiver link to (only the people who haven’t signed the waivers will be shown), add a message if you wish, and click “Send Emails.”


Player Medical Authorization Forms

League Policy asks that you carry medical authorization forms for all players on your team.  Medical authorization forms give a coach permission to seek medical care for a child in the parent's absence.  It also contains information about important medical conditions and allergies for the player.  Emergency medical providers will require a hard copy of this signed form to treat a child in a parent's absence (unless it is a life-threatening emergency).; There are two ways to get this form:

  1. If your team is recognized by your school administration, you are probably required to collect a school medical authorization form for all players. Inquire with your principal, athletic director, or activities coordinator.
  2. If your school does not provide a form, you can use this sample form provided by DiscNW. Note that there are two forms per page in order to minimize paper usage. Each player need only fill out a half sheet.

These forms will not be checked or collected by DiscNW. But for your own players' comfort and for your own protection, we trust you to carry them for all your players.


Check Waiver Status

You can check on your team’s waiver status by following the steps below. Players with a green thumbs up next to their name have signed all of the required waivers. Players who have a red thumbs down next to their name have not completed all of the required waivers. 

  1. Login to your discnw.org account.
  2. Go to the Spring HS Girls' League webpage.
  3. Click on “Registration” and select your team name in the drop-down menu.
  4. Make sure that you are on the “Waivers” tab. You will see your team roster with a thumbs up next to those who have signed the waivers, and a thumbs down next to those who haven't yet signed or completed the waivers.

If a player does not have online access to sign an electronic waiver, please see below for a paper copy.
