Paperwork:

The following paperwork is due by Wednesday, March 14

DiscNW prefers electronic copies of the paperwork, emailed to [Enable javascript to see email]. If you need to submit hard copies, please bring them to the coaches' meeting or mail to DiscNW at the address in the upper right corner of the website.

  1. The coach must submit a signed DiscNW Youth Participation Agreement and Release and Concussion Information Sheet for each player on the roster. Coaches should keep their own hard copy of the first page of this packet and bring it to all games and practices, since it contains a medical authorization which allows the coach to seek medical care for a player if the parent cannot be reached. Please alphabetize the player waivers by last name before submitting them.
  2. All coaches are required to sign a Coaching Code of Conduct form, which outlines coaches' responsibilities while participating in DiscNW events, and which signifies that they have been educated as to the nature and risk of concussions and head injuries.
  3. Each team must submit a team roster.  You may use the DiscNW Youth Team Roster, or the roster may be in your own format, but it must include the name of the event, the year of the event, the school and team names, and the full names, grades, and genders of all players. Please alphabetize the player names by last name on the form.

Division Rules:

  1. Schools with more than one team will automatically have one team assigned to the A pool. The other team(s) may play in any division, provided that the guidelines below are met. This ensures that the pools are all approximately the same size.
  2. Teams may be comprised of players from multiple grades.
  3. Teams with 8th graders may not play in the lowest division.
  4. Exceptions to the above rules may be granted with approval from the League Coordinator and Director of Youth Operations. However, teams who are granted exceptions to the division rules will not be eligible for the playoffs. For example, a brand new team with 8th graders may play in the lowest divison for the regular season, but it would eligible only for consolation games in post-season play (i.e., it could not compete for the division championship). To request an exception to the division rules, email .
